It means that the organizations may be lacking of Team Spirit, OR, lack of Problem Solving Skills in every employee, OR, Both.
Parable:
A car breaks down, 4 passengers, Repair Works cannot be done probably because:
(a) there is no knowledge of one, OR, all of the following:
- ANALYSING the root cause,
- what correct repair TOOLS to use,
- the step by step (METHOD) of the repair works, OR,
(b) There is knowledge but passengers are 'not motivated' OR have 'no team spirit' to execute the knowledge, which probably due to:
- mistakes in effort made are usually PUNISHED
- NO APPRECIATION if succeeded
II. SOME INSIDERS ARE BENEFITING FROM THE DECLINE (SABOTAGE)
Same parable (above):
That car can be sold as scrap with 1 or 2 of the passengers getting 'good commission' by it, whom will CONSISTENTLY discouraging repair works or even the idea of repairing.
Potential symptoms to this (only 'possibilities' unless proven otherwise) are:
- Delaying approval to improvement ideas / changes
- Sticking to defective-result venders/suppliers/consultant
- Bidding for a higher 'scrap sales' / 'let-go-assets sales' profit
III. ACTS OF GOD
Same parable (above):
All the car passengers died in the car due to an earthquake.
POTENTIAL SOLUTIONS - verify each potential causes whether or not they are true root causes and then, do a company-wide project to ELIMINATE all the root causes and PREVENT them from re-occurring once and for all.
